Job Summary:
The Resident Care Coordinator will play a crucial role in overseeing the care and support services provided to residents within our facility. This position requires a proactive, empathetic individual who is skilled in communication and problem-solving. The Resident Care Coordinator will work closely with residents, families, and staff to develop and implement tailored care plans, ensuring that all resident needs are met with dignity and respect.

Key Responsibilities:

- Assess and evaluate the needs of residents to create individualized care plans.
- Collaborate with healthcare professionals, caregivers, and family members to coordinate care services.
- Monitor residents’ progress and adjust care plans as necessary to meet changing needs.
- Ensure compliance with all regulatory standards and company policies regarding resident care.
- Provide training and support to caregiving staff regarding care protocols and best practices.
- Serve as a primary point of contact for residents and their families, addressing concerns and providing emotional support.
- Facilitate resident activities and social programs to enhance overall well-being and engagement.
- Document and maintain accurate records of care plans, resident assessments, and all communications with families and staff.
- Organize and lead resident care meetings and assessments.
- Advocate for residents’ rights and needs within the community.

Qualifications:

- Bachelor’s degree in Nursing, Social Work, Gerontology, or a related field. Relevant experience may substitute for education.
- Previous experience in a care coordination or management role, preferably in a residential care environment.
- Strong knowledge of regulatory requirements and best practices related to resident care.
- Excellent communication and interpersonal skills, with the ability to connect with residents, families, and staff.
- Strong organizational and time-management abilities, with a keen attention to detail.
- Compassionate and patient demeanor, with a commitment to advocating for residents’ well-being.
- Proficiency in standard office software, documentation systems, and resident management software.
